PIPD Update Feb 21, 2025

 

Lab labeling errors - please remember to double check that each and every specimen container being sent to the lab is properly labeled. We are seeing regular safety reports of unlabeled unit collect samples being received by the lab. These errors introduce increased risk of care delays and improperly recorded results, and create more work you when having to relabel. 

Caregiver/Visitor codes - we've had a flurry of incorrect visitor codes going out to caregivers and visitors making things confusing for all involved at the front desk. Just a reminder that it is the last 4-digits of the patient's visit number that is used for the code, not the MRN or any other number. 

Message from Parking Serviceselectrical plugs on any SHA parking facility (both surface lot or parkade) is for use of block heaters ONLY, as per the SHA provincial parking policy. It is strictly prohibited to plug in anything other than including electric vehicles. The electrical system is not equipped for the use of anything other than block heaters.

Managing Pyxis Discrepancies - This is a requirement from Health Canada
    Highlights:
        • All discrepancies are to be resolved by end of shift, this includes documenting any wastage.
        • Unresolved discrepancies at the end of the shift must be communicated the CNE.
    How do you resolve a discrepancy? Sign in to Pyxis. At the bottom of the screen is "discrepancies". You will need a second nurse to sign with you. You can select one of the pre-written options for why there is a discrepancy or write your own. Then hit "resolve".

Donation Kudos
With mom's permission I would like to give a kudos to Hudson, Tatum, Palmer, and Hayes for raising money at their Lemonade Stand to buy toys for the department. These 4 kiddos and mom showed up at the reception desk with 2 bags filled with toys 'to give to your patients'! It was super awesome to witness. 

Farewell PIPD Team
Thanks to ALL of you for making me feel so very welcome over the past few weeks and taking the time to chat with me, introduce yourselves to me and including me as part of your team. This has ben a great experience for me and has taught me so much about your department and given me a great understanding of your successes and challenges. Continue to be the supportive team that you are and I will see you around when I am given the opportunity to provide coverage from time to time.

CNE Updates

Female Curos Caps - are now found on the top shelf of your in-room Kanban! We will keep them in the med room drawer until a later time to allow everyone to get used to the change.

NEW Q-Cards

    Iron Transfusion - in your recipe box & on S-Drive. Just a reminder that if your patient needs an Iron Transfusion, contact Pharmacy for the Order Set. 

    NICU PICC - in your recipe box & on the S-Drive. Created in collaboration with the NICU CNEs. Pictures included on how to measure the length and an example of a dressing. Reminder to follow the Clinical Procedure and look at the Heplock/Flushing table. 
    Dressing changes are to be done PRN.
    Needleless Adapters (caps) are only to be changed when clinically indicated (cracked, leaking, etc).

Sleep Lab Work Standard

See attached a new WS for PIPD inpatients getting a sleep study in the Sleep Lab. A copy of this is in the CN binder and CN folder on the S drive. 

Transporting patients with trachs - reminder, when transporting a patient with a trach always bring portable suction with you. These are in Walmart and mini-Walmart. You can always refer to the trach Q-card which include this and other important reminders about trach care. 

Med Room Drawers

Unit 1, 3, 4 drawers have been tidied up. Working to complete Unit 2. Please take a peek and provide feedback

In Room Airway Boxes

Planning to start the transition on Feb. 25 of moving the NRB's to the outside of the airway boxes and remove the current airway bags from all patient rooms. Thank you to our support staff and CNE's for their prep work thus far to make this a smooth transition

PIPD Update Feb 10, 2025

 Kudos

The entire staff was amazing today (Feb 3). They supported each other and loved the little baby at the end of her life. I appreciate all of you and what our amazing staff/medical team does everyday - Dr Hodgson-Viden. 


What to do when a patient passes away on the unit - just a reminder for all staff, but especially newer charge nurses, that there is a reference binder in the unit 3 team room with the information you need to follow when there is a death on the unit. 

Scheduling:

- the volume of late trades and named replacements has increased recently and is a challenge for Charge Nurses in planning care, especially on weekends. These late changes come with the risk of scheduling and payroll errors so please plan an ahead and arrange these at least several days ahead of time. If you are doing a trade or named replacement <24hr in advance please notify one of the managers instead of the Charge Nurse as these will not be processed online in time for the shift. Please keep in mind that leveling rules apply. 

Parking

Parking Services is working on updating employee data including email address, phone numbers, employee ID’s, location, etc.

 

If you have applied for parking, please contact the parking office to ensure the information we have is accurate and up to date.

 

Any employee who parking services attempts to contact to offer parking via the email provided need to respond accordingly.

-          If you are not wanting to accept parking at this time we will need to have an email response from you either declining the parking or that you would like to stay on the waitlist.

-          If no response is received within 2 weeks of the email offering then you will be removed from the waitlist.

Airway Boxes

Thanks to all who participated in the survey. As per the results of the survey we will plan to keep BVM, HEPA filter, and code record in the airway box and the NRB's in a ziploc hanging off the side of the airway box. We will also be eliminating the airway bags that currently hang on the headwall as all these same supplies are on the Kanban carts

Food and Nutrition 

Friendly reminder to enter Temporary NPO’s for patients in 99 beds when you know they won't be here for their meal trays.

Door Code Changes

The door codes to the Food and Nutrition and Family Sleep room keypads have been changed:

Food&Nutrition new code is 1290# (same as phone # to call F&N)

Family Sleep room new code is 4537# (last 4 digits of CN cell phone)  

CNE Update

NEW Q-Cards
    Immunizations - in both the recipe box and the S-Drive. Has information on who can immunize and physician's responsibility with the ordering of vaccines.

    Trach Set-Up and Care - in both the recipe box and the S-Drive. It includes pictures of room set up on the back of the card.

UPDATED Q-Cards
    COVID Serology - in both the recipe box and the S-Drive.

    IM Injection Info - in both the recipe box and the S-Drive.

    Propranolol Monitoring for Hemangiomas - in both the recipe box and the S-Drive.

    Feeding Pump Information - about the Infinity pump. In both the recipe box and the S-Drive. QR code & link included on card for video on using pump.

    Healthy & Home Discharge Instructions - in both the recipe box and the S-Drive.

    Phototherapy Bili Blanket Bili Meter - in both the recipe box and the S-Drive. The Bili Meter card and Phototherapy Bili Blanket card have been combined to make 1 card. No more needing to look at 2 cards to get your set up done and care for your patient. Reminder - you need to chart the irradiance level in your patient's nursing progress notes when checking the level prior to starting phototherapy on the patient.

    Sternotomy Precautions - updated in collaboration with our Cardiology Nurses and Stollery guidelines. Pictures included on how to pick up infants and help older children move.

In reviewing the recipe boxes, Q-Cards were found that are not on the S-Drive. 
If you created Q-Cards, please send to the CNEs so we can create and add that valuable information to the Q-Cards in the correct format. This is important to ensure all information is up to date with SHA's best practice standards. 😄

Highlights you should know:

Biliblankets- an updated Irradiance Measurement log is hanging on each bili machine. 
    We only have 2 bili machines working at the moment. Replacement blankets have been ordered for the 2 machines currently out of order. 

    Blinatumomab - we have more CADD pumps in the Unit 3 Team Room! Each container has been labelled and contains: a CADD Pump, a charging cord, a key, and a pole clamp. 
    Each pump has also been programmed with a drug library for Blina! You can now choose your infusion length based on which bag has been ordered - 24hr, 72hr, & 96hr.
    In the front of Binder is an update to remind you we are now always using a clave on the end of the central line/port. Always review the front of the Blina Binder for Blina Updates when administering Blina.

RMHC Lunches

Please save lunches for our patient's caregivers only. It is a bad look when all we have to offer are ripped open bags to parents.
