Just a reminder or maybe you didn't know this if you are new: Any requests for information or medical records related to pt on the unit gets redirected to HIMS.

Any media requests get directed to SHA Communications.

Please do not disclose any information to anyone that is not listed as legal guardian or been cleared by manager (ie police).

 

Falls binder – thankfully we do not have a lot of falls. If you haven’t seen them, there are new Fall Protocol binders (bright green) in each unit to help guide staff on what to do to support fall prevention but as well, to guide you on what to do post falls for care and observation.

 

Safety Focus Groups are being held on August 5 and 6 at 9:00 a.m., 11:00 a.m., and 1:00 p.m.

These sessions are an opportunity to provide valuable input that will help improve the safety of staff, patients, and visitors across Saskatchewan healthcare facilities. If you are interested in participating, please sign up- details are in email below. We also encourage you to share this opportunity with members of your teams who may be interested. There are quite a few spaces still available.

• Safety of patients, visitors, and staff
• Hospital and health centre safety assessment(s)
• Current legislative and regulatory environment supports addressing hospital safety and security needs

 Did you know a Child life referral can be for the patient and the family? We didn't!

 We are lucky to have involved parents and sometimes they need a little extra from us. With a child life referral, they can help with coping plans, offer gift cards, and offer other resources that can help make the families stay more manageable. 

 Did you know if it's your patients birthday, you can go down to the gift shop and pick them out a present?

Jim Pattison Children's Hospital Foundation (JPCHF) is accepting grant requests until August 15.

JPCHF promotes excellence in maternal and pediatric health care across Saskatchewan, disbursing $4.5M in funding last year.

JPCHF grants patient care requests that show potential for enhancing care based on scientific excellence and relevance to children's and maternal health.

We encourage you to review our Granting Guidelines for more information.

We are looking at: Temp probes, new digital scale and highchairs. Anyone can apply and we can repeat asks to reinforce a need, just let Kelly or Sharon know if you do apply.

  CNE Update:

Nursing Progress Record Form # 101434 has been replaced by the new provincial form SHA 0021.  

SHA 0021 is to be used in its place moving forward.  

Here is an example of what the form looks like.  This has not been replaced in our Admission pack yet, however it will be coming in the near future.

 Reminder on how to Submit a 1600 for a Patient Safety Event:

If needing to submit a 1600, you can always call 1600 during the day.  

Online:  Go to SHA intranet - Forms & Policies - Patient Safety and Experience Forms - Click on Saskatoon Safety Reporting System (also known as AEMS)

Next you will see -SRS Prod Sign In.  Click on "Create Event"

Once you create an event, you will need to choose your Facility/Program.  Choose Royal University Hospital.  Then you will need to choose which category it belongs to.  Example Medication.  You will then need to choose which subtype.  

You will be prompted to fill in all of the spaces.  Have the patients chart handy as you need their first and last name. 

Thank-you for submitting a safety event.  This helps keep our patients safe and identifies areas for education, reinforced learning and process improvements.

What is a Job Shadow?

Job Shadows provide individuals an opportunity to observe an SHA employee within a healthcare environment as they carry out their daily duties. This experience will help individuals determine if the healthcare role fits their personality or their professional goals prior to entering a field of employment or education.

Who is eligible for opportunities to Job Shadow?

Grade 11 and 12 High School students;

Post-Secondary Students;

the community; and

current SHA employees.

 

What are the guidelines for a Job Shadow?

available in placements of 4 or 8 hour durations;

a one-time/single location placement opportunity (additional placements for hard to recruit positions may be considered); and available in a variety of areas/locations, but does not include physician shadowing or surgical/critical care placements including Emergency Departments.

 

Housekeeping Info:

 

1. IPAC spoke with EVS today regarding long stay cleans on the unit. They are happy to do a deeper room clean every 30 days for patients that have been admitted for long periods of time.  They ask that you notify their team by calling 2851, when these cleans are required and EVS will determine with the nurse when would be a good time for them to come. If you are able to move the patient out of the room during the clean- even better! Then they aren’t having to work around anyone.
2. It was brought to IPAC’s attention that staff are not seeing EVS clean high point areas in our unit- including the ceiling, lights, etc. Linda, The EVS manager will escalate. Their WS will be updated to ensure this is clear.  EVS should be cleaning these areas if noticeably dirty and for transfer/discharge cleans.
3. It was also brought to our attention that the area under the Kanban’s is not being cleaned regularly- this is something that should be done by EVS when they come do the daily room cleaning. If the pediatric staff are noticing that it is getting missed, Linda from EVS is encouraging them to mention it to EVS while up there. 

Pyxis Discrepancies:

Please note that if a discrepancy has occurred on your shift because you removed a medication from Pyxis without selecting it (lorazapam, clonazapam, epidurals), this is an error. If you remove a medication before doing count, that creates a count error. This is the email from Pharamcy:

This is another reminder that there are multiple outstanding narcotic discrepancies on your Pyxis stations.  You will notice some of them are >30 days outstanding and it is the responsibility of your department to resolve these.  Some of these date back as far as 91 days.  For best practice, discrepancies should be resolved at the end of every shift.  These records must be accurate to align with Health Canada legal requirements.  If you cannot get these figured out I will need to escalate to the next level, including reporting these to Health Canada.

 I have cleared off the discrepancies that Pharmacy created as of today, so any remaining discrepancies that have a Pharmacy staff linked to them will be because they found the error, not created it. A common theme for the remaining discrepancies is that your staff is taking the medication out when they are to be doing the blind count which creates a discrepancy in the current count and accessing the fridge other ways and not removing the medication appropriately in Pyxis to document usage.  This is also an increased risk to a patient for medication errors when medications are removed improperly and without proper documentation.  

** Please review the Pyxis machine in Unit 2 & 3 to see if your name is attached to a discrepancy. How to solve discrepancies: Sign in to Pyxis à go to “Discrepancies” on main screen à scroll down to the discrepancy with your name à select this discrepancy à click “Resolve” on bottom of screen (you will need another nurse to sign à choose a pre-typed reason for the discrepancy or type your own à select “Accept”
