PIPD Update Aug 23, 2024

KUDOS
Tracey and Carolyn for working extra hard to help manage capacity this week. It was very busy but every patient got a bed.

Privacy and Confidential Training is DUE END OF SEPTEMBER.
* please check your SHA email or previous blog posts for info.

Double Check Dosing
Please ensure you are double checking weight dosing of the physician orders. We have had some errors that could have been avoided with double checking the dose against the weight of the patient.

Antibiotic Resistant Organism (ARO) Screening- Required.

NEW to PIPD: we now a "High Risk Unit" as requested by the Peds Physician Team and directed by IPAC. What does this mean for nurses?
Starting Sept 3:
   1.  Continue to complete the ARO Screening Tool with each admission and transfer. We are "High Risk Unit" so you will need to answer 'yes' to #10 and swab for VRE & MRSA for all patients.
    2. If Infection Control comes by and writes an order for the extended stay (30 day) repeat swab, the swab must be completed.

Did you know? ARO screening and sending of ARO swabs is a Medical Directive! Not doing it is the same as not completing a physician order.

Handwritten MARs

Any handwritten MAR (ex: admission MAR, post op MAR) is to be double checked & double signature to prevent transcription errors.

Item out of Stock?

If any item is out of stock, please put in a 1600 and tell a CNE. CNEs do not always get alerted prior to items going out of stock. For example, 1mL ENFit syringes are not acceptable to leave out of stock.
If a CNE is not available, leave a message on Vocera - we will get the message when we sign in next and will follow up trying to get substitutes.

CNE Updates

Ed Days are: Oct 1, 2, 7, 23, 24, 25, 29, Nov 1, 4, 15
Dates have been sent to scheduling. If you need to trade Ed Days, please email a CNE with your date and your trade partner's date.

PCA Pump - if you are experiencing "Upstream Occlusion" or "Downstream Occlusion" but can't find the cause: clamp the PCA tubing, open the silver latch, remove the cassette, put the cassette back in, close latch, unclamp your tubing. If this does not fix it, try again and try to replace the pump by phoning PACU. The PCA pump needs to be sent to Clinical Engineering for service. Please put in a 1600 - a safety alert is the only way to track issues!

EVD Cart - If you or the NeuroSurgery Team have used supplies from the EVD cart, please fill in the Supply Form in the EVD binder and have the front desk order up the replacement supplies.

NEW 24g IVs - called SuperCath,; they are the exact same as our current 26g IVs. There is a box in each med room with a feedback form (easy ticksheet feedback). Please use these when starting IVs and let us know how they are. They are already stocked in Stores so we can easily replace the ViaValve with them if liked by staff. Your feedback will make change happen!

Translation and Interpretation Services: Your feedback matters

When using SHA Translation and Interpretation Services it is important to provide feedback. These valuable insights identify the areas where our services excel and where there is room for improvement.

You can submit feedback by visiting the Translation Services web page and using the “Submit feedback to translation services link” that creates a pre-populated email to gather your information.

A huge thank to the staff who have taken the time to complete the mandatory annual cybersecurity training! The annual cybersecurity training is part of the Saskatchewan Health Authority's ongoing efforts to enhance the security and safety of the province's health-care system. With the heightened risk we are facing worldwide, and the increased focus on health sector systems, now more than ever it is crucial that we are diligent in mitigating cyber risks.

This year's training is focused on home-based risks and password education. The training takes approximately 30-45 minutes, and must be completed by September 30, 2024.  You can s

PIPD Update Aug 2, 2024

CNE UPDATES:

EVD:  Our current EVD system is on backorder.  Please note that a new EVD system is now being used.  It is called the Integra AccuDrain.  All information is in the EVD binder in Unit 2.  Here is a video for your reference.  https://youtu.be/5eRKLgRQGps?si=ZI_r9WpKR8UepCmA

BLINA UPDATE:

1.       This week we have started another infusion of BLINA.  Due to the many reports of air in the line, we have started a NEW process of circle priming our tubing.  This means that the bag will come with tubing that is “dry/not primed.”    To circle prime, please follow the instructions in the BLINA binder.  Always use proper PPE, including a face shield.   We have asked pharmacy to have the tubing un-attached, but you may see the bag spiked with unprimed tubing.   Please note that this tubing will NOT be primed with drug from pharmacy.

2.       Circle Priming Video: https://www.youtube.com/watch?v=g7ovb1pwl_w

a.       Please note the Equashield connections and how to circle prime.  This video used a secondary set with roller lock, however we will be using a CADD pump prime function with CADD tubing.

3.       New to our unit is the infusion of BLINA into a single lumen port.  This is a change in practice for us and has some significant changes.  Patient will require a PIV for the first 4 days to ensure we have IV access if Port access is compromised.  Once the BLINA infusion has started, we will be assessing the port site to ensure patency and not be drawing back blood when doing daily bag changes – unless clinically needed for assessment of port by RN.  All blood will be done peripherally except during port needle changes.  Please refer to BLINA binder for new process and instructions.

Upcoming Preceptors:

If you have a 4th year student in the fall, we will have a Preceptor handbook for you.  This includes all the information you need to pass to your student like common medications, diagnoses, ward standards, medication quiz,  a skills checklist for your student, and other helpful info.  Please talk to a CNE if you need one.  They will be ready for pick up over the next few weeks.  Thanks so much for sharing your wisdom, tips and patience with the students.

Alteplase: Reminder to complete your CVAD learning, and sign the book by the managers office.  Also, ONLY IF you are an obs trained RN, attended 2023 ED days, and have completed your learning – you can now administer Alteplase.
