Weekly Update May 12, 2023

PICU à PIPD transfers – ongoing and recent challenges with the handover of patients coming out of PICU has led to a new handover process for all transfers. We’ve worked with PICU on this for the last month and trialed the process for two transfers, one complex and one straightforward, and had really positive results. Therefore, we are implementing the process effective immediately (see attached WS). The main difference is that we will go to do handover in PICU and bring the patient back with them. This allows for better communication at the start and opportunities to see the patient as part of that handover. We are in discussions with NICU as well about doing a similar process with them in the future.

Dietician charting change MAY 15 – starting May 15 RDs will be charting in SCM in three documents: 1) NFS RD Nutrition Assessment 2) NFS RD Progress Note – Pediatrics 3) NFS RD Nutrition Support Flowsheet.

PIPD will be participating in an Implanted Vascular Access Device Trail.  Any patients getting a new port will be a part of this trialThere will be evaluation forms in the Charge nurse room and we are asking that only experienced staff access and de-access these ports and help participate in the evaluation and share their experience.  More details to come next week.

JPCH Bronchiolitis Pulse Oximetry Monitoring Protocol Survey – for the trial bronchiolitis protocol used this winter, here is a quick evaluation survey we would love to get staff feedback on - https://redcap.rqhealth.ca/apps/surveys/?s=XXKF8H37HF9JYAWH

Please find attached the work standard for accessing Occupational Therapy Services on the weekends and statuary holidays. This will take effective, May 13, 2023.  Please share with your teams and physicians as appropriate.

LPNs – Your CEP chart has been saved into the S-Drive under LPNAC’s & Scope of Practice. Thank you Danielle for sharing!

Ondansetron disintegrating tab is out of stock. See attached PDF. Ondansetron oral (liquid) solution is available.

Please welcome our 12 new staff who have started orientation. To our Charge Nurses & Resource nurses - we know there are a lot of new staff to think about so we have created a (hopefully) easier way to plan the buddy and independent shifts. There are clipboards hanging on the wall in the Charge Room. Please see the attached picture. We hope this helps

Saskatchewan Health Authority Patient Label Standardization Survey

The purpose of the survey is to gather and assess valuable input from areas whose work is directly impacted by patient labels. Provincial standardization will establish a uniform patient label to be used across the Saskatchewan Health Authority (SHA), instead of regional label differences between our former health regions.

Completing the survey will take approximately one to three minutes, and responses are confidential. The survey link will be available from May 9 to June 9.

Please feel free to forward the Patient Label Standardized survey link to front-line staff.

 Weekly Update May 19,2023

Tracy Schwab has accepted the PFT clinical coordinator position, starting June 5. Tracy has worked in pediatrics for many years, we are happy to have her back on PIPD!

Privacy Training 2023

As employees and physicians within the health-care system, we are entrusted to protect the personal health information of the patients, clients and residents we serve, as well as the personal information of our Saskatchewan Health Authority (SHA) employees. As per the Health Information Privacy Act (HIPA), we have a legal obligation to ensure their information is protected, and only accessed when required for care, or when permitted by legislation. The Local Authority of Freedom of Information and Protection of Privacy Act (LA FOIP) provides the right of privacy with respect to the personal information we hold about our employees.

To ensure all employees understand their roles and responsibilities when it comes to protecting patient and employee privacy, the SHA requires all employees and physicians to receive training in privacy and confidentiality on a regular basis.

The Privacy Office has launched Privacy Training 2023 in MyConnection. The previous privacy training module in eLearn has been discontinued. Paper copies of the quizzes will no longer be accepted.

All employees and physicians are required to complete this training by September 30, 2023. Visit the SHA Privacy intranet page for more information.

 Weekly Update May 26, 2023

Lorrie A’s retirement – there is a little farewell book for Lorrie in the small staff room. Her last day of work will be June 14.

Travel history and IPC guidelines – reminder to always include a travel history in your admission assessment and document. Do not assume this is done in the absence of documentation. Asking this question will aid you to screen for any routine infectious disease risks (i.e. CPO as part of ARO screening) but also higher-risk infrequently seen diseases (i.e. Monkeypox & Ebola). There are Infection Prevention & Control resources to refer to once you know a travel history to guide next steps, just do a simple search on the SHA intranet or bookmark this page into your browser or save the link to your desktop - Infection Prevention and Control (IPAC) | SaskHealthAuthority. The Guidelines and Toolkits section has specific information on things like Monkeypox and Ebola.

Starting May 29, 2023 at 0800 for all admissions coming from CES only, SFCC will no longer be receiving calls to “call-in” patients for admission. Physicians/residents have been made aware that instead of relying on the Coordinator/Charge Nurse to call-in the bed, they will now need to complete an Admission Order Form which the CES UA will then enter into Allscripts Patient Flow. This is a process change affecting all of the Saskatoon hospitals and is not optional. There have been concerns with this process change and its effect on providers escalated to the System Flow dept. This form will need to be completed before an admission can proceed. We have asked physicians to please not contact the Coordinator or Charge Nurse to admit instead of completing this form, as you will not be able to do this.

Please note that the option of Low Sodium Diet (3-5gm) has moved from the Dietary Modifications section of all diet forms to the Sodium Restriction section of the order form- see attachment

Hand Hygiene Audit May 2023 65%. Handwashing was missed:

-          Entering and exiting a patient room and being in contact with a patient

-          After being in contact with a patient

We need to be better. Let’s get above 80% next time.

New Q-Card for nursing! It’s on the S-Drive under Q-cards Nursing – Client Representative (Patient Advocate). See attached picture. This way you have the information if you are unable to find the tear-away notepads with the same information.

PIPD will be participating in an Implanted Vascular Access Device Trial.  Any patients getting a new port will be a part of this trialWe currently have 2 patient on the unit with this new port.  They will have a bright green poster in their care plan with instructions.  Please fill out evaluation after each use.  These ports have no practice change.  Photocopy more evaluation forms, or find them in the bright blue binder in the CN room.  We are asking that only experienced staff access and de-access these ports and help participate in the evaluation and share their experience.   Thanks

KUDOS:

-          Sarah J taking sick admit like a champ

-          Caitlyn for being the best resource nurse- so helpful!

-          Karen, Libebunch & Heather for running off their feet short staffed on this very busy Friday!

 Weekly Update June 2, 2023

Sending Faxes- When you are faxing to a 655 #, you only need to add the last 4 digits- I.E our PIPD fax # 6475 (9-306-655-6475).  Anytime you are adding 306 it has to be 9-306, if you are sending fax long distance it has to be 9-1-306.  If the numbers aren’t done properly, I.E same as our unit phones they will come back failed.  Please make sure before you are sending.  IT has suggested unplugging machine for 1-2 minutes to re-boot or call them so they can deal with it in the moment- please do so! If they’re still not working please let us know, and save your fax failed copies.  Thanks

IVIG has an updated order set for pediatrics: CS-OS-1911 PEDIATRIC 10% Intravenous Immune Globulin (IVIG) Order Set (saskhealthauthority.ca).

This will be live starting June 1 and is found on Forms on Demand. Any patient receiving IVIG must have this order set filled out by the physician.

PORT ACCESS CONCERN: Day medicine had an issue with a micro-gripper port needle malfunction. If this happens to you – please save the malfunctioned port needle AND the package it came in (place the needle device into an orange top container) for the CNE. Preferably fill out this form as much as you are able: https://www.saskhealthauthority.ca/system/files/2022-10/Form-SupplyChain-ProductIssue.pdf or leave us a very detailed note so we can fill it out.

Nurses – it is your responsibility to REMOVE ALL IV tubing, bags, blood products, chemotherapy from the patient’s IV pole and pumps! If your patient is discharged – you MUST remove and properly dispose of those items! This is not the scope or role of the support staff.

Chemo Course – June 20 & 21: For those of you taking this course, an email with more details will be coming out next Monday. LEARNING PACKAGES are available in the Charge Nurse Room – the package has a sticky note with your name on it. It is available for pick up as of May 31.

BFI Pre-Assessment June 27: A BFI minded lady will be coming for a walk through to see our space and maybe ask staff some questions. Big things to remember:

    1. Breastfeeding is welcome anytime, anywhere!
    2. A breastfeeding parent asks for a private area to pump/breastfeed – main floor by the JPCH Foundation office
    3. WE SUPPORT THE FAMILY NO MATTER HOW THEY CHOOSE TO FEED!
    4. Look for the fun poster with pink information sheets at every unit you can review starting June 2. These documents have been included in this email. It is also in the BFI folder on the S-drive.
    5. LASTLY – there is a Feeding POLICY now! It is something you may get asked about where to find it: It is on the S-Drive – BFI folder. It is called “JPCH BFI Guidelines 2023 (POLICY)”. Don’t let the “guidelines” title fool you – it is still a POLICY. It is our pledge to support families.

Super Mario Week next week: please see attachment

Magic Mouthwash has been discontinued. Once the current supply is used up, the physicians will need to order a different option. See the attached PDF memo.

Maternity Leave – if anyone has questions about maternity/paternity leave, there is a bunch of info in MyConnection. Simply search ‘Maternity Leave’ in MyConnection for a list of resources. The document titled 'HRBP Maternity/Parental Leave' has a lot of helpful information. Some documents are attached.

 Weekly Update June 16, 2023

Thank you to everyone who contributed to Lorries retirement gift/party on Thursday. She was touched by the gift and wanted to pass this along.

To my exceptional co-workers present day and from years gone by,

I am humbled by all my friends and co-workers, physicians and residents and Interprofessional staff that came to the coffee party today to share good wishes and memories. I am overwhelmed by your thoughtfulness and generosity with the gift basket items and gift cards and all the well wishes in my retirement book. Good thing I don't have far to drive home as I was crying, mostly happy tears, on the way. Retirement will be an adjustment but I know the ward is in very capable and caring hands. I will stop in from time to time or see you at a coffee shop somewhere. Take care of each other!

New to Q-cards on the S-drive:  IV recommendations for CT procedures.   Please review if you have a patient needing a CT scan- see attachment

Effective June 13, 2023, the Infection Prevention and Control (IPAC) Progress Note will be available on SCM:  This structured note will be utilized by the Infection Prevention and Control Practitioners for any data information pertaining to new referrals and ongoing care. See attached PDF.

June 27 is fast approaching for the BFI Pre-Assessment. If you are working that day, please review the laminated pink information sheets available in each unit. The Assessor will be touring the ward and may ask ANY staff member questions. You already do family centered care so you got this!

This week’s info tips:

    1. The Feeding Policy is called “JPCH BFI Guidelines”. A copy of it is in the BFI folder on the Acute Care Pediatrics S-Drive
    2. Breast/chest feeding is welcome to happen anywhere the parent feels comfortable. If the parent wants a quiet/private room to feed or pump, there is a room on main floor by the public washrooms (by the JPCH Foundation office).
    3. If the parent is here with the patient AND has a sibling in the room they are nursing – please support the parent. Ensure they know breastfeeding is welcome anytime. Offer a pump if needed & let them know they can store breastmilk in the patient fridge.

Hello all,

I am conducting a survey to look at the attitudes of our pediatrics team towards patients/children with obesity. I would really appreciate if you could take the time to complete the survey. It should take less than 10 minutes to complete.

https://www.surveymonkey.ca/r/F2PZZTQ

Your cooperation is greatly appreciated. Please feel free to email me at nita.chauhan@usask.ca if you have any questions/comments/concerns.

Many thanks,

Dr. Nita Chauhan

Good morning from RMHC-SK!

We value our partnership with our healthcare providers, and would love the opportunity to share who we are, who we serve, and how we can support the model of Family Centered Care through accommodations.

On Thursday, July 7th, we invite you to stop in at the House for our healthcare appreciation breakfast and tour! For those who are unable to join in person, there is an online opportunity that same afternoon from 2:00pm-3:00pm.

Please share with your team, and RSVP to myself if you’re interested in joining.

 Weekly Update July 21, 2023

Hydrocortisone shortage:

There is currently a hydrocortisone shortage. If your patient has hydrocortisone ordered for any reason other than septic shock or adrenal insufficiency please advise the physician ordering that it is not available at this time and to order a suitable alternative.

Methylprednisolone should be ordered on the pediatric reaction order set instead of hydrocortisone.

Please see attached memo.

Resource nurses – an updated ‘Facilities Census with Diagnosis’ (short census list) will be printed daily at 1830 along with a the longer one for Charge Nurses so that the resource nurse has an update patient list for the night shift.

Label 100ml NS bags with 30 day expiration date once large bag opened: It has been brought to our attention that once the large bag is opened then the 100ml bags inside expire in 30 days once opened. Please label the bags remaining using the date opened stickers. Continue to check the bag expiry date as well. See attached safety alert.

KUDOS

Everyone that worked on July 21st day!! Especially Carly and Syndey for extending to help in the morning!! GREAT TEAM WORK!!

 Weekly Update July 7, 2023

RN/LPN Certification Records:  If you have done training in the last year (Orientation or Chemo), your certification records are outside the Managers office in the CNE basket.

Last dose on MARs – reminder that all MAR notifications with “last day/dose” for specific medications need to be flagged for the medical teams to review.

PD supplies – all PD machines and supplies go to and from storage room 2013, not the hemodialysis room.

Congenital Syphilis Public Health Process – all original orders for Pen G and daily MARs must be faxed to public health. MARs need to be faxed at midnight as Public Health Tracks doses given for the entire 10 day course of treatment. If any doses are missed a 10-day course must be restarted. There is a Q-card with these details and fax number in the S drive.

Night nurse chemotherapy checks:

Chemo checks are done by two chemo trained nurses on nights. Chemo checks are done the first night that chemotherapy orders are written. They check the BSA, correct drug and dosage of chemotherapy, the roadmap and MAR against the original orders. Subsequent day of chemo infusion the night nurse does the usual MAR to MAR comparison.

Can I give an antibiotic in a 3ml syringe through a central line? Yes you can. When flushing with NS we must use a 10 ml syringe as we are using a stop start technique to clear the line. However, you can administer an IV antibiotic in a 3ml syringe over 3-5 minutes as you are pushing the med slowly and not exerting the same amount of pressure on the line as you do with a NS flush. There is a higher risk of cross contamination transferring antibiotics from one syringe to another than there is of damaging the line with a slow IV push medication in a 3ml syringe.

Transfusion Medicine Education Needs survey – please send transfusion med your thoughts on education needs https://redcap.usask.ca/redcap/surveys/?s=JLL7HE83KX4NRRDD

All employees must complete the mandatory privacy training module by September 30, 2023. This is not the same as the Cyber Security Training that was completed earlier this year.
